2. SNP Food Forms-all
3. Child’s Birth Certificate
4. Child’s Social Security Card
5. Child’s Immunization Record
6. Both Parents Driver’s License
7. Health Insurance Card
8. Getting to know your child and home practices forms
9. Custody paperwork-if needed- (Please include pictures of anyone NOT authorized for pick up)

We encourage you to include pictures of direct family members with your preschool or daycare enrollment application.
